TRANSFER TO NEW NAME (CHANGE NAME)

• Authorization letter from the previous owner will be required to allow the new applicant to change ownership, the new owner will affix his or her signature personally upon filling up change name form to officially transfer ownership of service connection.
• If ever there is an outstanding account it should be settled first.
• Bring proof of previous Billing for identification.
• Fill up Change or Transfer Name Form
• Pay the necessary amount for Change or Transfer name.
• The new transfer’s name will appear on next billing.
